How do I become eligible for treatment at Algoma Sports Medicine?

You will require a referral from your physician.  Please have your family doctor complete the clinics' referral form by clicking the button and bringing it with you to your next appointment with your provider.  

Once we receive your referral form, a referral appointment will be forwarded to your family doctor with all of the details of your first visit to our clinic.
